EOP and Letters of Recommendation
Letters of Recommendation Forms and Guidelines
Please submit a completed "My High School Student Profile" form along with either "Student Information Worksheet" or "Me Personally Worksheet" below to the person writing your letter of recommendation. If you applying to a CSU or UC and are applying for EOP, please complete the EOP form and also give this information to your counselor.
